To Whom It May Concern

Howard G. Kirtley

RR 2 Box 298

Milton, WV 25541

DOB: 09-07-1943

We were in St. Mary’s Hospital from 07:00 PM Tuesday 11-29-11 thru 05:00 PM  Friday 12-02-11.

On Thursday 12-01-11 our Hemoglobin count was down to 5. We were given 3 units of blood in the evening hours of Thursday 12-01-11 then another unit of blood in the morning hours of Friday

12-02-11 and we were discharged from the hospital that same day.

We were going to do a out patient Colonoscopy and Gastroscopy with Dr Mark Subik at Three Gables Surgery Center in Proctorville Ohio. The schedule date was to be set the following week

We came back to St Mary’s Hospital ( out patient ) on Dr Mark Subik’s orders on Saturday 12-10-11 and got 2 more units of blood making it a total of 6 units in all.

We were scheduled for a Colonoscopy and Gastroscopy on

Monday 12-12-11 at 07:30 AM.

We arrived at Three Gables Surgery Center on Monday 12-12-11 at 7:00 AM, after having gone through the 36 hour ordeal of no food, 11 DulcoLax Tablets and 8 doses of MiraLax.

At 8:15 AM we were ( informed by the receptionist ) that it would be at least 10:30 AM or some time later in the day ( if ever ) before the procedure would take place. Also to add more wait time for me, the lady said that there were 2 people ahead of me.

How could 2 people be ahead of me when my appointment was at 7:30 AM ?

And can you believe this........

We were asked if we wanted to reschedule the Colonoscopy and Gastroscopy.

That would be another 36 hour ordeal of no food, 11 DulcoLax Tablets and 8 doses of MiraLax.

I have never been so completely dumbfounded and speechless. ( I was so…… tired )

Dr Subik never made an appearance of anykind to explain anything about the delay or what was the cause or how long it would be. “NOTHING”

My wife and I left with no intention of ever going back to Dr. Subik or Three Gables Surgery Center.

The next day, Tuesday 12-13-2011 I went to HIMG and got an appointment with Family Practice Doctor Stephen L. Sebert, MD. Dr. Sebert is now my Doctor of record. Dr. Sebert then referred me to Dr, Matthew A. Rohrbach ( Gastroenterology ) for a Colonoscopy and Gastroscopy.

We got an appointment with Dr Rohrbach (on Tuesday 01-17-2012) to get the Colonoscopy and Gastroscopy performed. Our appointment was at 08:45am. At 08:40am I was called into the procedure room for the Colonoscopy and Gastroscopy to be performed. At 10:04am my wife and I were pulling out of HIMG's parking lot going home. That is only 1 hour & 24 minutes to get a Colonoscopy and Gastroscopy over and done with and be pulling out of the parking lot.

NOW THAT'S THE WAY TO DO BUSINESS.

We are 100% satisfied with Dr. Stephen L. Sebert MD, Dr. Matthew A. Rohrbach MD, David George PA-C and all the staff at HIMG.
